Fill in a Session Observation Form

Mark a recorded session against the ICF competency model, then export ICF's Session Observation Form with every rating, piece of evidence and feedback point already filled in.

New to the forms? Read the overview first for how RaeNotes decides each rating.

Step-by-step guide

1

Choose the competency model

Open the recording and go to the Rubric tab. Pick the model of the credential level: ACC MSR with BARS (2025), MCC MSR with BARS (2025) or Minimum Skills Requirements for PCC (2025). The Rubric tab also lists every competency and statement to look for.

2

Mark the moments and rate each statement

Select the words in the transcript, choose the competency and the statement they demonstrate, and, for ACC and MCC, a rating: Exceeds Standard, Meets Standard, Below Standard, Does Not Meet Standard or N/A. Add a note to say what you observed — it becomes the comment under the evidence on the form. See Tag competencies and markers for the basics.

  • Mark the same statement as often as it happens, at the rating each moment deserves.
  • For Competency 1, mark statement 1.1 (alignment with the ICF Code of Ethics) and 1.2 (staying in the role of the coach) with a rating. They tick the form's qualifiers Q1 and Q2.

3

Check the preview in the Scores tab

The Scores tab shows what the form will say: each statement's rating and each competency's two-thirds verdict. See Preview the form in the Scores tab.

4

Open Export and choose the ICF template

Click Export at the top of the recording. Keep Microsoft Word (*.docx) and choose the template of the recording's level, such as ICF ACC Session Observation Form. Templates for other levels are greyed out.

5

Set the export options

  • Include pre-filled assessment form — tick it; the form is filled from your markings.
  • Statement ratings resolution (ACC and MCC) — how a statement marked more than once is rated: Most frequent rating (default), Lowest or Highest. It is the same setting as in the Scores tab; changing it in one place changes both.
  • Select overall strengths and opportunities — tick it to pick the feedback examples in the next step.
  • Export specific notes authors — leave All, or choose whose markings to include.

RaeNotes remembers these options for your next export.

Pick strengths and development opportunities

Click Next. Drag moments from Comments into Overall Strength or Overall Development Opportunities.

  • A strength is shown on the form when the moment is evidence of the statement's rating.
  • A development opportunity may be any moment, such as one rated Below Standard while the statement as a whole meets the standard.

Click Download.

Review the form in Word

The downloaded document is ICF's form with RaeNotes' answers filled in:

  • Competency 1: Q1 and Q2 ticked from 1.1 and 1.2, and the Comments listing every Competency 1 moment with its quote and your note.
  • Competencies 3 to 8: one rating ticked per statement, with the moments behind it as Evidence of Behavior. A statement you did not rate keeps ICF's empty box.
  • Observation Feedback: the strengths and development opportunities you picked, each with the statement, the quote and your note.
  • The session summary, if there is one, and the transcript.

The checkboxes stay Word checkboxes and the text stays editable, so you can adjust anything before you sign. A few parts are left for you, such as the reflections, the level of the mentor coaching services and the signature.

Good to know

  • No template in the list? The ICF templates appear for recordings marked against one of the three models above. Contact us if they are missing.
  • A statement with no rating. A statement marked without a rating — for example by RaeNotes' AI, which marks statements without a rating — shows no rating and no evidence until you rate it.
  • PCC. The PCC form ticks Observed or Not Observed for each marker following ICF's PCC rubric, so there is no Statement ratings resolution to choose.
